By accepting the terms of the JETPACK LOGISTICS LLC. and its affiliates, (Referred to as the Service Provider in this document) you hereby acknowledge that you have read, understood, and agree to the following:

• Your packages may be transited through one or more countries during its journey before arriving at the final address and that Jetpack logistics LLC and its affiliates may substitute air and ground carriers without prior notification or approval from you as customer.

• Packages received in Miami without proper identification, with hazardous materials or for holders of closed accounts may be put on hold until their status changes to one where they are cleared for forwarding and, that any package placed on hold, is not the responsibility of Jetpack Logistics LLC nor its affiliates. JetPack Logistics LLC reserves the right to dispose of any held package after 30 days on hold.

• The address being provided is solely for the purpose of receiving packages and mail and to facilitate the forwarding of said packages and mail to your specified final destination. The Customer has no rights to the physical address.

• All packages deemed as "restricted articles", "hazardous material", or "dangerous goods" by: Department of Transportation (DOT), International Air Transportation Association (IATA), OR International Civil Aviation Organization (ICAO), will be held at the Miami Location until further information is provided by the customer. All such packages will be discarded after a period of thirty (30) days and neither JetPack Logistics LLC nor its affiliates will accept responsibility nor liability in such instances. All associated cost associated with further delivery instructions provided by the customer for packages regarded as "restricted articles", "hazardous material", or "dangerous goods" will be borne by the customer.

• The shipment may, at our option or at the request of governmental authorities, be opened and inspected by such authorities or us at any time.

• Unless the option is declined by the customer, all packages will be covered by and where necessary, charged for insurance unless the customer chooses to opt out of the insurance program. Waiving Insurance explicitly releases the JetPack Logistics LLC of all responsibility or liability due to loss or damage of any packages. Insurance fees are calculated and charged by our system and are based on the declared value of the shipment. Failure to declare a value may lead to other calculation methods. The service provider reserves the right to cancel the Insurance service for any account without prior notice. There are certain items that are not insurable under our insurance program and the customer will be notified before forwarding.

• JetPack Logistics LLC and its affiliates assume responsibility when the packages are received at the warehouse in the Miami facility. The Service Provider will not be held liable for packages that are lost, delayed or damaged while in transit to the warehouse. Shipments will be consolidated and shipped to the destination country based on pre-established schedules and under the limitations of available space on Air carrier’s aircraft. The service provider cannot be held liable for any shipments not accommodated by the air carrier on any given day but will optimize operations to decrease this possibility.

• The Service Provider will refuse any item shipped to its address damaged or opened and will notify the consignee immediately. No claims will be accepted nor honored by the Service Provider with respect to these items, and the Service Provider will have no liability or responsibility. Claims must be filed in writing within 30 days from the date of shipment from the service provider.

• Insurance only covers the declared value of an item and does not cover shipping costs, handling fees, customs duties or any other cost associated with the transportation or receipt of the item.

• We make no warranties, express, or implied.

• JetPack Logistics LLC is not liable for delays in delivery, any resultant inconvenience, losses or disruptions. The Service Provider will not be liable for the customer’s acts or omissions, including but not limited to incorrect declaration of goods, improper or insufficient packaging or marking or addressing of the shipment, or for acts or omissions by the consignor or consignee or anyone else with an interest in the shipment. Also, the Service Provider will not be liable if you or the consignor violates any of the terms of the agreement.

• JetPack Logistics LLC will not be held liable for loss, damage, or delay caused by events outside of our control including, but not limited to natural disaster, Government agency or police action.

• Any Charges associated with the receipt of any unsolicited mail to your account will be your responsibility. JetPack Logistics LLC will not be responsible for cancelling any subscriptions or revolving shipments.

• You agree to pay all charges imposed by the government and/or agency, and/or the carrier with respect to your mail and packages.

• You agree to pay any customs clearance charges imposed by JetPack Logistics LLC and its affiliates with respect to your packages.

• Freight charges are calculated based on actual total weight of each package

• We reserve the right to adjust our charges to reflect any changes in the cost of providing our service.

• All packages must be paid for on delivery. By accepting these terms and conditions, you agree that all charges for mail or packages will automatically be charged to the debit or credit card on file. JetPack Logistics LLC and its affiliates may debit the accounts maintain the right to hold the customer’s shipment in lieu of payment and sell any packages on hold to cover any associated outstanding charges.

• All credit card information provided on the online sign-up form is current and true and any changes thereafter will be provided promptly to avoided packages being held. The service provider also reserves the right to reverse a transaction at its discretion and notify the customer of the outstanding balance if the card provided cannot cover the receivable and hold the packages pending alternate payment arrangements.

• The email address provided at sign-up or updated thereafter will be used for the delivery of all forms of correspondence. These include but are not limited to: Invoices, notifications, direct marketing, suspension, and cancellations notices and tracking updates.

• This agreement can be terminated by giving thirty (30) days’ notice in writing or by making the required selection from the customer’s online account page.
  Any charges related to packages received after cancellation date are still expected to be paid.

• JetPack Logistics LLC Inc. and its affiliates reserves the right to revoke, alter, add to or amend any of the Terms and Conditions, in addition to any service, features, or rate without prior notice.




CONSENT TO SEARCH

• In accordance with the Transportation Security administration Indirect Air Carrier Standard Security Program dated July 31,2010 Part 2.2.H.1 I hereby consent to the screening of all cargo shipped through Jetpack Logistics LLC under this account from this day forth. I am aware that this agreement will be retained on file by Jetpack Logistics LLC and will be subject to Inspection by the TSA, air carriers, and/or any government agency.

• I certify that we will not attempt to ship via Jetpack Logistics LLC any unauthorized explosives, incendiaries, dangerous goods, or hazardous materials
